Answer to Case Study #2:

Work with Mary to come up with a strategy to protect herself. Explore the dynamics of her relationship. What is the extent of the abuse she lives with? How does she feel? Is leaving her husband an option? Does she think that her husband would be willing to come to the clinic to speak to a counselor? Explain the source of her infection, how to treat it, and how to use condoms to prevent further infection. Give Mary a condom demonstration, and have her practice putting a condom on a penis model. Discuss various strategies for talking about condom use with her husband, if she thinks that it is possible. Role play how she might broach the subject with her husband. If female condoms are available in your area, discuss with Mary the possibility of her using them.

Explain the importance of having her husband get treated because if left untreated, syphilis can lead to serious complications, even death. (Explore the possibility that her husband may have already sought treatment at another clinic, when his sores became apparent, but emphasize that if he did not, he is still infected with syphilis even if the sores are absent.) If facilities exist in your area, provide a referral to Mary for counseling regarding her abusive relationship.
