Answer to Case Study #1:

Given the above description, it does not appear that providing ARVs would be an effective use of the clinic's resources. The challenges involved in providing ARVs in low-resource settings go beyond the costs of drugs: a strong infrastructure is required for their correct application and the monitoring of clients using the drugs. Some ARVs require refrigeration, which the clinic could not rely upon given the problems with the electricity. Clients taking ARVs would need a reliable supply of clean water to take the medication (and perhaps to avoid side effects of certain ARVs, such as kidney stones) and reliable access to a facility for routine lab work needed for monitoring the health of people using ARVs-neither of which is available. It seems that the clinic would better utilize its limited resources by channeling them into prevention activities rather than treatment.
